Now Available: EH FHIR eCQM Draft Measure Packages for 2025 Reporting/Performance Period Public Comment
The Centers for Medicare & Medicaid Services (CMS) invites vendors and interested parties to review and provide feedback on draft specifications for two Fast Healthcare Interoperability Resources (FHIR) electronic clinical quality measures (eCQMs): CMS506 Safe Use of Opioids and CMS1028 Severe Obstetric Complications. CMS506 and CMS1028 are measures developed in MADiE and are currently operating under QI-Core STU 4.1.1.
Your feedback will enable CMS to learn from electronic health record (EHR) vendors who have the technical capabilities to (1) review the draft FHIR measures in the Clinical Quality Language (CQL) standard for logic expression and (2) test the code by directly absorbing machine-readable XML files for eCQMs based on the FHIR standard (if feasible). The testing will help CMS identify instances in which the XML code produces errors and resolve those errors, as well as identify issues with the QI Core profile to further advance these evolving standards. Users’ comments on the potential impact of EH FHIR eCQMs will help CMS and the measure stewards make improvements to minimize provider and vendor burden in collecting, capturing, calculating, and reporting eCQMs. User’s feedback on CMS506 Safe Use of Opioids and CMS1028 Severe Obstetric Complications draft specifications will be used to investigate any necessary changes to measure maintenance processes or standards. These updates will be communicated to measure developers, ensuring all EH measures are revised as needed.
The draft measures in HTML, XML, and JSON formats will be available June 4, 2024, through July 5, 2024, in the ONC Project Tracking System (Jira) eCQM Measure Tracker. See the following master ticket:

Refer to the QI Core 4.1.1 to better understand how QI Core works to provide eCQMs that are human readable yet structured for electronic processing. Measures will not be eligible for reporting unless and until they are proposed and finalized through notice-and-comment rulemaking for each applicable program.
