Hospital Quality Reporting Program Quarterly Measure Data Submission

Facilities participating in the Centers for Medicare & Medicaid Services’ (CMS’) Hospital Outpatient Quality Reporting (OQR) Program should be aware of upcoming data submission deadlines:

For the Quarter (Q)4 2025 reporting period (October 1, 2025 —December 31, 2025): 

• Data for the clinical measures OP-18: Median Time from Emergency Department (ED) Arrival to ED Departure for Discharged ED Patients and OP-23: Head Computed Tomography (CT) or Magnetic Resonance Imaging (MRI) Scan Results for Acute Ischemic Stroke or Hemorrhagic Stroke Patients Who Received Head CT for MRI Scan Interpretation Within 45 Minutes of Arrival submitted via the Hospital Quality Reporting (HQR) system and are due by May 1, 2026, at 11:59 p.m. Pacific Time (PT).
  • Facilities can update and correct their submitted clinical data until the submission deadline. After the deadline, the HQR system will be locked. No further data can be submitted, and no changes can be made. 
  • The submission of Hospital OQR Population and Sampling data is voluntary.

For the calendar year 2025 reporting period (January 1 —December 31, 2025):

• Data for the web-based measures OP-22: Left Without Being Seen and OP-29: Appropriate Follow-up Interval for Normal Colonoscopy in Average Risk Patients submitted via the HQR  system are due by May 15, 2026, at 11:59 p.m. PT.
• Data for the Electronic Clinical Quality Measure OP-40: ST Elevation Myocardial Infarction (STEMI) submitted via the HQR system are due by May 15, 2026, at 11:59 p.m. PT.
  • Hospitals must submit two self-selected quarters.
