Updated Hospital-Specific Reports for Hybrid HWR and HWM Measures Now Available
The Centers for Medicare and Medicaid Services (CMS) would like to notify users that updated Hybrid Hospital Wide Readmission (HWR) and Hybrid Hospital Wide Mortality (HWM) Hospital-Specific Reports, computed using claims, voluntary CCDEs and linking variables, are now available on the Measure Details Dashboard in the Hospital Quality Reporting (HQR) System.
CCDE values and corrected percentages of successfully linked vital signs and laboratory test results are now available.
As the HQR System continues to improve, any changes will be communicated.
