CMS Releases Hospital-Specific Reports for the Severe Obstetric Complications and Patient Safety Indicator Measures that will be Publicly Reported in Fall 2025

Review your Hospital-Specific Reports (HSRs) on the Hospital Quality Reporting (HQR) system, for the Severe Obstetric Complications Electronic Clinical Quality Measure (eCQM) (PC-07) and CMS Patient Safety Indicators (PSI) measures that will be publicly reported in Fall 2025.

The Preview Period began on August 5, 2025 and will end on September 3, 2025.

CMS encourages hospitals to review their HSRs and submit questions about their calculations as soon as possible. Submit all requests for corrections using the QualityNet Question and Answer Tool no later than 11:59 p.m. PT on September 3, 2025. More information about the preview period process is available in the HSR User Guides available within the HQR system and on the applicable measures’ page on QualityNet.

The HSRs include hospital-level performance data displayed on the Measure detail dashboard of the Hospital Quality Reporting (HQR) system, as well as downloadable comma-separated values (CSV) files with additional details and your hospital’s patient-level data. Hospital-level performance data is also available in downloadable portable document format (PDF) files. Each report has an accompanying user guide with information about the contents of the HSRs.

The following measures are included in the reports:

  • Patient Safety Measures
    • CMS PSI 04 – Death Rate Among Surgical Inpatients with Serious Treatable Complications
    • CMS PSI 90 – Patient Safety and Adverse Events Composite, calculated based on ten component PSIs [qualitynet.cms.gov].
  • Maternal Health Measures
    • PC-07 – Severe Obstetric Complications Electronic Clinical Quality Measure (eCQM), includes two outcomes:
      • Severe obstetric complications
      • Severe obstetric complications excluding delivery hospitalizations for which a blood transfusion was the only numerator event.

CMS will no longer confidentially report facilities’ results stratified by race or ethnicity and payer type nor will it report the disparity methods calculations for race or ethnicity and payer type.  

The Cesarean Birth eCQM (PC-02), that will be publicly reported in Fall 2025, will not be included in the Measure Detail dashboard due to the removal of stratified results and disparity methods calculations. Instead, hospitals should refer to the Public Reporting Preview Report or the eCQM Performance Summary Report.
