Review CY 2025 Hospital eCQM Data Mapping and Submissions

To prepare for the upcoming electronic clinical quality measure (eCQM) data submission deadline of March 2, 2026, please:

1. Review your Quality Reporting Document Architecture (QRDA) Category I files. 
2. Follow the technical specifications outlined on the eCQI Resource Center. 
3. Closely review your measure data results in CMS’ Hospital Quality Reporting (HQR) System before attesting that the information is accurate. 

Errors in the QRDA Category I files submitted to CMS may lead to inaccurate scores. CMS encourages careful review and would like to note that we have received previous QRDA Category I files with the following common errors:

1. Incorrect mapping, the mapping of incorrect codes in the electronic health record (EHR) to measure value sets
2. Duplicate encounters 
3. Missing key data elements, such as the present on admission (POA) indicator for measures, if applicable

Please note that CMS cannot accommodate correction requests made after submission deadlines.

CMS encourages hospitals to:

1. Validate QRDA Category I test file submissions and correct files prior to production file submissions.
2. Submit eCQM data to the HQR System early in the data submission period.
3. Review messages in the HQR System during data submission.
4. Review observed rates and submitted eCQM data to identify and correct any errors.  
5. Review hospital-level measure information, specifications, and data elements located on the eCQI Resource Center website.
