Now Available: 2025 CMS QRDA III IG, Schematron, and Sample Files for EC Programs
The Centers for Medicare & Medicaid Services (CMS) has published the 2025 CMS Quality Reporting Document Architecture (QRDA) Category III Implementation Guide (IG), Schematron, and Sample Files for Eligible Clinician Programs. The 2025 CMS QRDA III IG outlines requirements for Eligible Clinicians (EC) to report electronic clinical quality measures for the calendar year 2025 EC quality reporting programs.
The 2025 CMS QRDA III IG contains these high-level changes compared with the 2024 CMS QRDA III IG:
- Added support for the Making Care Primary (MCP) model.
- Updated to require the CMS Electronic Health Record (EHR) Certification ID for the Merit-Based Incentive Payment System (MIPS) Quality performance category from optional.
- Updated the QRDA Category III Report – CMS template from V8 to V9 to support the MCP model and the CMS EHR Certification ID requirement change for the MIPS Quality performance category.
- Updated the UUID List table to include the MIPS CY2025 performance period eCQM specifications for ECs.
The 2025 CMS QRDA III Schematron File contains these high-level changes compared with the 2024 CMS QRDA III Schematron File:
- Updated to support conformance statement updates as outlined in this version of the IG.
The 2025 CMS QRDA III Sample Files contains these high-level changes compared with the 2024 CMS QRDA III Sample Files:
- Added MCP sample file.
- Updated to support updates as outlined in this version of the IG.
