Hybrid Measure Outcomes Page Now Available in HQR

A new Outcomes page for Hybrid measures is now available in the Hospital Quality Reporting (HQR) System. The Outcomes page will display core clinical data elements (CCDE) data elements for Hybrid measures.

The eCQM permission for Inpatient Quality Reporting (IQR) is required to access the Outcomes page. If you do not have this permission, you can request it by navigating to the My Profile page and creating a new access request.

To access the Outcomes page, log into HQR with your HARP ID.
1.    Navigate to Data Results on the left navigation menu and select Hybrid Measures.
2.    Select the third tab for Outcome.
3.    Select the appropriate Submission Type, Quarter, and Measure.
4.    Click Change Selection and the report will display the appropriate CCDE data.
