Please join us for this one-day conference and Connectathon to bring together diverse stakeholders to learn about and discuss key issues in making patient-centered and patient-facing clinical decision support a reality. During the course of this one day conference, EHR vendor representatives and demonstration application developers will be available to assist with connecting (via SMART on FHIR or similar) apps to EHR portals. This event is open to attendees and will be held in a small breakout room adjacent to the main conference hall. MM/DD/YYYY
