Each year, CMS updates the electronic clinical quality measures (eCQMs) for potential use in CMS quality reporting programs and publishes them on the eCQI Resource Center. CMS updates the specifications annually to align with current clinical guidelines and code systems so they remain relevant and actionable within the clinical care setting. CMS requires the use of the most current version of the eCQMs as specified and intended for the applicable reporting periods for all quality reporting programs.

Hospitals are to use the updated eCQMs to electronically report 2026 clinical quality measure data for CMS quality reporting programs.

For hospital inpatient reporting guidance, including the Inpatient Quality Reporting Program, Medicare PPS-Exempt Cancer Hospital Quality Reporting (PCHQR) Program or the Transforming Episode Accountability Model (TEAM) visit QualityNet, the Quality Reporting Center or submit questions to the QualityNet Question and Answer Site.

For Medicare Promoting Interoperability Program guidance, refer to the CMS Promoting Interoperability Program webpage.

For Medicaid Promoting Interoperability guidance, refer to your state Medicaid agency, https://www.medicaid.gov.

Submit questions on the technical aspects of eCQMs to the ASTP/ONC Project Tracking System (Jira) eCQM Issue Tracker.
