Measure Description:

This logic is intended to extract electronic clinical data. It is designed to extract the first captured set of vital signs and basic laboratory results obtained on adult Medicare fee-for-service patients admitted to acute care short stay hospitals. These data will be linked with administrative claims data to risk-adjust hospital-level hybrid outcome measures. This is not an electronic clinical quality measure and this logic will not produce measure results. Instead it will produce a file containing the data that CMS will use to risk adjust the hybrid HWR measure.

Prophylactic Antibiotic Received Within One Hour Prior to Surgical Incision

Measure Description:

Surgical patients with prophylactic antibiotics initiated within one hour prior to surgical incision. Patients who received vancomycin or a fluoroquinolone for prophylactic antibiotics should have the antibiotics initiated within two hours prior to surgical incision. Due to the longer infusion time required for vancomycin or a fluoroquinolone, it is acceptable to start these antibiotics within two hours prior to incision time.

Depression Remission at Twelve Months

Measure Description:

Adult patients age 18 and older with major depression or dysthymia and an initial PHQ-9 score > 9 who demonstrate remission at twelve months defined as PHQ-9 score less than 5. This measure applies to both patients with newly diagnosed and existing depression whose current PHQ-9 score indicates a need for treatment
