CMS’ Hospital Quality Reporting system is now open for submission

CMS’ Hospital Quality Reporting (HQR) system is now open for the mandatory submission of EHR-derived clinical data and linking variables for the fiscal year (FY) 2028 Hybrid Hospital-Wide Readmission (HWR) measure and Hybrid Hospital-Wide (HWM) measure for the July 1, 2025, through June 30, 2026, reporting period. The data submission deadline is Thursday, October 1, 2026, at 11:59 p.m. Pacific Time.

The Centers for Medicare & Medicaid Services (CMS) has updated the HQR system to accept Quality Reporting Document Architecture Category I test and production files associated with this reporting period. The requirements are located on the Hospital - Inpatient (Hybrid) page.
