Reminder: FY 2027 Hybrid HWR and Hybrid HWM Measure Data Due October 1, 2025
Hospitals participating in the Hospital Inpatient Quality Reporting Program that voluntarily report core clinical data elements and linking variables for the Hybrid Hospital-Wide All-Cause Readmission (HWR) measure data and Hybrid Hospital-Wide All-Cause Risk Standardized Mortality (HWM) measure data have a submission deadline of Wednesday, October 1, 2025, at 11:59 p.m. Pacific Time.
