Hybrid Measure Data Due October 1, 2026

Hospitals participating in the Hospital Inpatient Quality Reporting Program are required to report EHR-derived clinical data and linking variables for the Hybrid Hospital-Wide Readmission (HWR) measure and Hybrid Hospital-Wide (HWM) measure by the submission deadline on Thursday, October 1, 2026, at 11:59 p.m. Pacific Time.
