CY 2016 eCQM Reporting Updates for the IQR and Medicare EHR Incentive Programs
QualityNet Secure Portal No Longer Accepting Submissions for CY 2016 eCQM Reporting
The QualityNet Secure Portal is no longer accepting electronic Clinical Quality Measure (eCQM) reporting for Calendar Year (CY) 2016 reporting for the Hospital Inpatient Quality Reporting (IQR) or the Medicare Electronic Health Record (EHR) Incentive Programs with the close of the submission period on March 13, 2017. The Centers for Medicare & Medicaid Services (CMS) will notify hospitals and their vendors when the system will be available for CY 2017 data submission.
EHR Hospital Reports Available in the QualityNet Secure Portal for CY 2016 Reporting
Effective immediately, hospitals and their vendors are able to generate the EHR Hospital Reports through the QualityNet Secure Portal to assess outcomes and begin process improvement activities based on CY 2016 eCQM reporting to the Hospital IQR and EHR Incentive Programs.
IQR-Eligible Hospitals May File CY 2016 eCQM ECE Requests Through April 1, 2017
IQR-eligible hospitals may file Extraordinary Circumstances Exemption (ECE) requestsrelated to CY 2016 eCQM reporting through April 1, 2017. Please follow the instructions on the Extraordinary Circumstances (ECE) Request Form tab of the Electronic Clinical Quality Measures (eCQMs) Reporting section of QualityNet to ensure the timely submission of the ECE Request Form before the April 1, 2017, deadline.
Note: Hospitals that have previously submitted an ECE request for CY 2016 eCQM reporting and were initially denied may submit a new ECE request with the most current supporting documentation for CY 2016 reporting. All supporting documentation must be provided at the time the ECE request form is submitted.
CMS Issues Updated eCQM ECE Policy Clarification Questions and Answers
CMS has issued an updated list of questions and answers to help clarify the ECE policy for eCQM reporting for the Hospital IQR Program. The updated ECE Policy Clarification Questions and Answers can also be found on the Extraordinary Circumstances (ECE) Request Form page.
2018 EH Hardship Instructions and Form
The deadline for Eligible Hospitals (EHs) to submit Hardship Exception forms for the 2018 payment adjustment, based on the 2016 EHR reporting period, is July 1, 2017. Application instructions and the application form are available from the CMS.gov website as follows:

The deadline for Critical Access Hospitals (CAHs) to submit Hardship Exception forms for the 2016 payment adjustment, based on the 2014 EHR reporting period, is July 1, 2017. Application instructions and the application form are available from the CMS.gov website as follows:
EH and CAH Attestation for Medicare EHR Incentive Program for CY 2016
Hospitals that submitted their CQMs electronically will see “Pending eReporting” now that the submission period has closed, and the reporting outcomes are transmitted. For hospitals, this will occur around late March or early April 2017.
The prior eCQM status of “Pending eReporting” will be replaced by one of two messages: “Locked for Payment” or “Expired.” If the message reads “Locked for Payment,” the Medicare EHR Incentive Program is indicating that the provider completed the electronic reporting option for CQMs successfully. If the message reads “Expired,” the message is indicating that the provider did not complete the CQM electronic reporting option successfully.
